When joining the club, please print out all of the forms and return the top four, completed,  to us along with the 10.00 joining fee (cheques payable to Chislehurst Millennium Swim Squads). The standing order form should be given to your bank ( or you could set it up online ).

Your forms need to be processed by us and then submitted to the ASA. Until you have completed the forms and standing order and are registered with the ASA you will not be covered by their insurance and will not be able to swim.
